Instructions:
Step1: Click on below link, and select OptionA (top left option) to login using your GMM number and password

Step2: Upload the documents.
Note: Please Do Not give your Credit Card number on form for Payment. In text box just write:
"Paid at Center"
Step3: Submit form and download once form is successfully Submitted.
Step4: Print out the form and sign at the given space for Student Signature. Print out the documents uploaded (1 passport size photo and proof of DOB)
Step5: Submit Fee + hard copy of Signed Form (above) + Photo + Proof of DOB latest by Feb 10th.
